How To Sign UP

  1. Login into your account 
  2. Once you are logged in and your membership is active you should see the Appointment link in the main menu.
  3. Click the appointment link that will redirect you to our appointment engine.

  1. Choose the appointment type
  2. Select the date and time of your preference.

  1. Login into your passport account or fill out the form as a Guest.
  2. Click continue to confirm your appointment.

  1. Login into your account.
  2. Once you are logged in click the link “My Account” located at the top rigth corner.
  3. Under your account click Payment Methods.
  • You can text or call our office (254) 327-1800 
  • If you received an appointment reminder you can cancel o reschedule your appointment in the link provided.
